Real-world failures in critical thinking

Theranos

Theranos, a company that promised to revolutionise the blood testing industry with a new technology, suffered from the sunk cost fallacy and confirmation bias. The company's founder, Elizabeth Holmes, disregarded early warning signs and evidence that the technology was not working as promised. This resulted in a costly legal battle and the loss of more than $600 million in Theranos investments.

2008 Financial Crisis

The 2008 financial crisis was caused in part by anchoring bias among mortgage lenders, who relied too heavily on real estate appraisals as the anchor for loan amounts. This led to the widespread issuance of risky, high-value loans, which ultimately led to widespread foreclosures and financial losses when the housing market collapsed.

Challenger Shuttle Disaster

The Space Shuttle Challenger disaster was a tragic event that resulted in the loss of seven crew members. Hindsight bias played a role in the disaster, as NASA engineers and decision-makers believed after the fact that the explosion was predictable and could have been prevented with better engineering and risk assessment. However, at the time, they did not consider the potential consequences of the cold weather affecting the O-ring seals, leading to a decision to launch the shuttle despite known risks.
